If you have had him scraped for Demodex I would suspect it is a Staph infection or he could be allergic to the food you are feeding. These things can resolve themselves without rushing to the vet. Do the spots have scabs or pus. You can try the original brown colored Listerine to clean and use Gold Bond Powder, I have a couple different types of astringent antibiotic sprays I use on things like these.

and it doesn't typically happen over the back of the dog. It is most common on the sides and the skin darkens. It is not in round spots but large patches.
If it is simply hairloss and no staph, bacteria, or demodex then you might want to consider the food you are feeding and/or start a supplement like Missing Link.
Both times my bullie had surgery, she ended up losing her back hair in round spots, differing sizes. She was put on antibiotics and it always took two rounds. I also got advice on here to use Animox lotion. Not sure what worked, but her fur is back to beautiful!
